Occupational Therapy 

Occupational therapy program evaluates and treats the "whole" person, including physical and psychosocial wellness. Our customized OT programs incorporate activities, exercise, and physical agent modalities to restore, maintain, develop, and adapt tasks to reduce pain and improve each client's functional independence in performing activities such as:

  • Utilizing upper extremities for work-related tasks

  • Fine motor skills (i.e., writing, buttoning, typing)

  • Grooming/hygiene

  • Dressing

  • Reaching overhead

  • Sensory modulation and sensory integration

Physical Therapy 

Our customized treatment is programmed to meet each client's distinctive needs. Our goal is to restore, maintain, encourage, and adapt movement/dysfunction to promote optimal wellness through exercise, pain reduction, and physical agent modalities. PT treatments, for adults, seniors, and children may address such conditions as:

Medical Social Services

A medical social worker assists patients with difficult medical situations by providing the support and resources they need to recover from their injury or medical illness. These professionals can also be referred to as clinical and healthcare social workers.

Social workers support patients dealing with emotional, physical, or psychological issues and help improve the patient’s quality of life.

To begin, medical social workers assess the patient’s needs by:

  • Evaluating the situation to determine the type and extent of help the patient needs.

  • Collaborating with the family, other health care providers, and colleagues.

  • Organizing the support a patient will need to recover or regain health

Speech Therapy 

Evolve Home Health speech and language pathology program focuses on the client's difficulty with verbal and/ or written communication. It involves diagnosis, assessment and treatment of physical and / or cognitive deficits/ disorders concentrating on improving: 

  • Speech (articulation, intonation, rate, and intensity)

  • Language (phonology, morphology, syntax, semantics, pragmatics, and receptive and expressive language such as reading and writing).

Who Needs Medical Social Services?

The following groups of patients may benefit from medical social services:

  • The injured

  • Terminally ill

  • The homeless

  • Recovering patients

  • The disabled

  • People with emotional, social, or financial problems

It’s important to note that virtually any individual can benefit from medical social services. Each medical social service will be personalized to the needs of the individual patient.

Home Health Aid Services

If the family caregiver is no longer able to keep up with caregiving responsibilities, it may be time for the help of a home health aide. Signs that a loved one is no longer able to live alone at home include:

  • Leaving the stove turned on

  • Difficulty preparing simple meals

  • Poor eating habits or nutrition

  • Forgetting to bathe regularly

  • Wandering away from home

  • Unsafe driving habits

  • Frequent falls or having difficulty walking without help

  • Forgetting to go to the doctors’ offices or other appointments

Some pushback is normal. Many older individuals hate to lose their independence and admit that they need help. However, there comes a time when a loved one is no longer safe on their own. It’s important to discuss various care options before determining the right approach to home care.
